New acute care functionality, Intelligent Transitions, gives hospital care managers the tools and data to track and manage patients as they leave their facility and move across the care continuum. Through a direct feed with the hospital’s EHR, Intelligent Transitions can alert the hospital if a patient discharged from their facility in the past 30 days is now having an ED, Inpatient, or SNF encounter. This allows the hospital to support the patient in their transitions of care and provide interventions to prevent readmissions and network leakage.
Why Intelligent Transitions?
• CMS has developed programs that penalize hospitals and cut reimbursement to hospitals with high rates of readmissions. Moreover, patients may get readmitted to a different facility, which results in lost revenue for the hospital.
• Hospitals are entering risk arrangements with health plans and ACOs to manage readmissions and transitions of care for a specific patient population. Therefore, meeting their quality outcomes now has implications to a hospital’s bottom line.
Hospitals have struggled to gain timely and accurate visibility into the patient’s care after they are discharged from the hospital. Traditionally, hospitals have had to rely on outdated claims data that doesn’t provide any ability for the hospital to intervene when a patient presents back to an ED, Hospital, or SNF.
